Research Participant Feedback Form

You (or someone you represent) recently participated in research at the BSI. We very much appreciate hearing about your (or her/his) experiences as a participant. You can choose to fill out the form anonymously by not including your name, e-mail address, and phone number. However, if you have any questions or concerns about a BSI study and would like to be contacted by a representative of the BSI, please make sure to include your contact details (e-mail address and/or phone number).

Who participated in a BSI study? *

Prior to your participation, you should have been well-informed - directly by the researcher(s) or indirectly by a collaborating party (e.g., school, health care institute) about the study and its purpose, you should have received the opportunity to ask questions, and any questions you had should have been answered by the researcher to your satisfaction.

In some studies, it is necessary to debrief or provide feedback to the participants after their participation.
