Request approval for programme

The Examination Board has to approve the courses you wish to follow in your programme. These need to be in accordance with the Education and Examination Regulations (EER) and the application must be submitted to the Examination Board three months before the exam date. Individual adjustments to the program must also be approved by the Examination Board in advance of a graduation request (also in connection with awarding a distinction).

You can download the application form below. Save the form as a PDF document, check whether all information is legible and then send it to the Examination Board by email: fnwi.examcies@science.ru.nl. You are responsible for submitting a complete application form; so please check that no information disappeared converting the document from Excel to PDF. Adjust the print area if necessary. Only forms that are sent by e-mail to the address above will be processed. Mention your name, your student number, your study and the phase of your study in the e-mail. Mention your name and your student number in the subject line of the e-mail. Make sure that the application for approval of your program contains the correct course codes and course names. Course codes and course names can be found in the study prospectus, for example via the course finder. If you submit an incomplete form, it cannot be processed. If necessary for the decision-making, the Examination Board will review your study progress.
