My research has its focus on hemodynamic physiology (systemic and cerebral) in aging, vascular disease, and cognitive decline (including Alzheimer's disease). One example is how blood pressure affects the brain through its effects on cerebral blood flow. The brain has complex systems that aim to dynamically adapt brain blood flow to changes in supply and demand (e.g. changes in blood pressure, metabolism or neuronal activation). My expertise is in measurements of cerebral perfusion using near-infrared spectroscopy (NIRS), transcranial Doppler ultrasonography (TCD) and perfusion MRI, integrated with continuous blood pressure measurements using Finapres.
Using these techniques, I study changes in brain perfusion associated with aging, and with two of the most important diseases in aging: hypertension and Alzheimer’s disease. Methods include dynamic cerebral autoregulation, vasomotor reactivity to CO2 and functional Near Infrared Spectroscopy.
Examples of clinical applications of my research are to use hemodynamic physiology to personalise patient management and treatment. For example, I study how treatment of hypertension affects brain blood flow in a patient with Alzheimer, or whether such a treatment will cause orthostatic hypotension.
Currently, I am also interested in applying these methods to optimise the outcome for older patients with complex vascular disease, such as abdominal or thoracic aneurysm.
